Information you submit is used and shared to provide your care, including with:

• —Your treating clinician and care team within Weightstry’s affiliated medical practice

• —Business associates (such as our technology and electronic health record providers) under written agreements requiring them to safeguard it

Independent pharmacies and laboratories maintain their own privacy policies for the services they provide. How your PHI is used and disclosed for treatment, payment, and operations is described in our HIPAA Notice of Privacy Practices.

Weightstry, LLC does not sell, rent, or trade personal information.

We use cookies for site functionality, analytics, and advertising measurement.

Weightstry’s affiliated medical practice is a HIPAA-covered entity. Weightstry, LLC provides administrative and technology services to the practice and handles PHI as its business associate under a Business Associate Agreement.

How your PHI is used, disclosed, and protected — and the rights you hold over it — is described in our HIPAA Notice of Privacy Practices.

Services are not offered to residents of the following jurisdictions: Delaware, Arkansas, Kansas, Louisiana, Mississippi, Rhode Island, New Mexico, West Virginia, District of Columbia.

We do not knowingly collect or process personal information from individuals located in these jurisdictions.

Our services are not intended for individuals under 18.

We are not responsible for third-party websites or their privacy practices.
